Make an Appointment

I1. Download Intake Form. and then fax or email to receptionist (receptionist@anklefootdoctor.com)
2. Call office: 603-543-FOOT(3668)
3. Cancellation Policy: there is a $25 “no show” charge for appointments not cancelled within 24 hours.

Please complete the enclosed Intake Form and bring it with you to the appointment.  If it is your first visit to our office, kindly plan to arrive fifteen minutes early.   Please be sure to bring your insurance card and any co-payment you may have.  Co-payments will be accepted in the form of cash, check, or credit/debit card. If you have records from other physicians or hospitals, x-rays, MRI’s or laboratory studies, please bring this information.  If you have arranged for records to be sent, please contact us ahead of time to confirm that our office has received them.

A “No Show” for a scheduled appointment is a serious burden to our practice and unfair to other patients waiting/wanting to be seen earlier.  If you are unable to keep your appointment,  kindly give our office at least 24 hours notice.  We will charge a $25.00 fee for a missed appointment.  Thank you for your courtesy.
